1. What is the manner of making complaints to Ombudsman?
A. Any person may make a complaint to the Ombudsman within Three Months from the date on which person making a complaint come to know of alleged violation. Ombudsman may extend time period by another Three Months if satisfied by prevalent circumstances.
B. Form -A & Informed consent may be down loaded from the website of the office of the Ombudsman (gsacsonlione.org ). All complaints shall be made in “Form-A. Informed consent to be submitted along with filled Form-A.
DAPCU, ART , ART PLUS , LINK ART, CSCs, DLN/SLN, DSDG, NGO’s/CBO’s & ICTC Staffs are directed to help illiterate Complainants to download, fill up Form – A and Informed consent.
C. Ombudsman will receive the complaints made in person/ by post/through Email or official-website/ Orally / Telephonically. Provided that complaint received orally or telephonically shall be later scribed by the complainant or his representative in Form -A.
